Hello. Welcome!! We don’t know about you, but we are SO EXCITED for this!! We think we’re all going to be thrilled with how this new practice is going to work. We are, however, asking for flexibility and patience as we get up and going as we are sure there is going to be unforeseen issues. We are trying to anticipate any concerns, but you know life!!
When you click on the following link, it will bring you to our electronic medical record (EMR) Enrollment Form. Our EMR is called Atlas.MD. ATLAS.MD IS A SECURE SITE. It is safe to enter personal information here. The form is NOT something that we can modify. Fill it out as best as you can. Many of the options are limited in their answer options. There are some things that we don’t offer. If it doesn’t make sense to you, just skip it and ask us later.
There’s a lot about medical history on this form. If you can’t fill it out or don’t have the time, we’ll fill in the details later. Although, the more you can do the more it helps us! The only thing that you HAVE to enter is your name, date of birth and contact information (I think).
If you are planning on paying by credit card or by debit card, you can enter in the billing info on this form. Again, this is a secure site. If you are planning on paying by check or cash, you simply don’t fill out the credit/debit card info.
For gender, please enter how you want us to address you.
You will need to enter an email address if you want to save the info then finish the form later. So, if you don’t have an email, give yourself at least 15min to complete the form out completely.
SMS means text messaging.
We don’t use twitter, so don’t worry about entering a Twitter account name if you have one.
Even though we are not getting paid by insurance, we still need your insurance and prescription card info on file. This is so we can process authorizations and make referrals for you.
You will be able to review the Patient Agreement. In order to enroll, you will need to check that you have read and agree to the Agreement. This is our contract. We will have you sign a hard copy of it when you come in to see us at your first visit.
In the Agreement, there is a section that talks about consenting to letting us communicate with you though potentially nonsecure methods. We want to explain this, so you’re informed and not scared by what this means. We will take all standard precautions to keep your information safe and secure. ANYTHING IN YOUR MEDICAL RECORD (meaning in our EMR, Atlas.MD) IS SAFE AND SECURE TO THE HIGHEST STANDARDS AVAILABLE. HOWEVER, communicating via email, Facebook, skype, and text may not be secure. Someone, like a hacker, can gain access much easier to these methods. They would be breaking the law to do so. It is not like someone would “happen upon” your emails or texts as we use standard protection (encryption and passwords, etc).
By giving consent to let us communicate with you via these POTENTIALLY nonsecure means is what makes communicating easier, we just need to be careful about what information we put on those communications. For example, don’t send us any personal info, like your credit card #, social security # or passwords to anything by email or text. Also, very delicate and sensitive medical issues need to be discussed by phone or in person. Any questions, let us know.
